Prior to beginning any therapy it's important that I, the therapist, have available to me background information on my client. By having you provide this information prior to our meeting it prevents us from spending an entire session just gathering information and allows me time, prior to our meeting, to formulate and decide on the direction we might go in therapy.

Intake Form ~ Adult

If you will be seeing me for individual therapy please complete this intake form prior to our first meeting.  Just fill it out online and click "submit".  It will be sent directly to me.

Limits of Confidentiality

Your therapist will protect your privacy and cannot release your information without your consent.  There are some exceptions to this. This document explains the exceptions. I will have this form at our first meeting and will review it with you.

If you are serving, or have served, in our military please complete this intake form. This will give me a better understanding of when and where you have served and your role in our military.  When you have completed the form just click "submit".  It will be sent directly to me. Thank you!

This HIPAA document explains your privacy rights.  It's important that you read it. I will provide this at our first meeting and will review it with you.
